Fidelis Dual Advantage Flex (HMO D-SNP) By Fidelis Legacy Plan Fidelis Dual Advantage Flex (HMO D-SNP) is a 2020 Medicare Advantage Special Needs Plan by Fidelis Legacy Plan. This Fidelis Legacy Plan plan works with Medicare to provide significant coverage beyond original Medicare. If you decide to register, you still retain Original Medicare. But you will get the additional coverage of Part A (Hospital Insurance) and Part B (Doctor Insurance) of the Fidelis Legacy Plan and not the Original Medicare. All Medicare SNPs also provide additional Medicare Part-D drug coverage. Fidelis Dual Advantage Flex (HMO D-SNP) BA-H3328 is a dual-range special needs plan (D-SNP). An eligible dual NPP is for beneficiaries who are eligible for Medicare and Medicaid. Part D Premium Supplement is $0.00 this Premium covers any improved benefit of the plan offered by Fidelis Legacy Plan above and beyond the standard PDP benefits. This may include additional coverage in the gap, reduction of collateral payments and coverage of non-Part D medicines. Part D Total Premium is $27.10. Part D Total Premium is the addition of supplemental and basic premiums for some plans that this amount may be less due to basic or supplemental negative premiums. Premium AssistanceDepending on your income level you can be eligible for 75%, 50%, 25% premium assistance. The Fidelis Dual Advantage Flex Medicare Insurance Plan (HMO D-SNP) offers a premium obligation of $0.00 if it receives full support from low-income grants (LIS).
